Safe Environment Information and Forms

Any employees or volunteers of St. Charles Borromeo Parish, or it's affilate organizations, such as the CYO, the Cub Scouts or the Boy Scouts need to complete the Archdoicese of Philadelphia "Safe Environment" training program.

To complete these, you must submit to a PA State Criminal Background check, a child abuse clearance check, and attend a Safe Environment training course.

The two State forms you will need to fill out, are attached to this document at the bottom.

Home and School Voluteer Form

This is the Home and School Volunteer Form

Private Dentist Report form

This is the standard Dentists report that all incoming students need to have filled out.

Private Physician Report form

This is the standard Physician's Health Report form all students need before they start the school year.  You will need Adobe Reader to open this form.
